The medicine is prescribed based on the doctor's evaluation and insurance company formulary preferences. We prescribe Subutex to only two groups of patients without exception: Pregnant and Self-pay. No exceptions will be made.
You may need to stay on Suboxone at least for several months for the treatment to be successful. The length of time is different for different people. Suddenly stopping Suboxone is not recommended. Once you have remained clean from opioids for several months on Suboxone, you may discuss with your doctor about weaning it off or reducing the dose. However please remember that everyone is different and some people may need to remain on Suboxone for several years. We try to give the minimum dose that is necessary. Most people need the maximum dose recommended as below
*Zubsolv 5.7mg, two a day (equivalent to Suboxone 8mg) Suboxone or Subutex 8mg, two a day
Many Health Plans require certain documentation to approve the medication. This process frequently requires your Toxicology Tests and Proof that you are going to Therapy. Without the Auth, your meds will not be paid for by the insurance company.
Authorization is time consuming and while you are waiting for this, the best option is to go to the pharmacy ask them to Partial Fill your prescription and pay for as many pills as you can afford to start or continue the treatment. In general Pharmacists are fantastic at working with patients in need. Talk to them and see if they can help. You will not get this money back once you receive your authorization from your insurance company.
Buprenorphine is the active medication in all these medications. Subutex only contains Buprenorphine. All of the other products (Suboxone films/tablets, Zubsolv, and Bunavail) are combination products and have Buprenorphine and Naloxone. Naloxone will only activate when injected or snorted.
Cognitive therapy is very important for successful treatment of opioid addiction, Most importantly, your Health Plan probably requires you to do therapy with a licensed Drug and Alcohol Therapist. Although every plan is different, most plans mandate therapy. In some cases such as self pay cases we also accept documented AA/NA meetings.
Benzodiazepine use is absolutely not permitted. If Benzodiazepines are prescribed to you by a physician, then please talk to your physician about tapering off. If you can wean off your Benzodiazepine safely then you should begin to do that. If Benzodiazepines are present 60 days after initial consultation, immediate discharge will occur.
Amphetamines, Adderall, Ritalin, Weed, sleeping pills etc. use is not permitted. The policy is the same as Benzodiazepines.
We reserve the right to terminate you without cause. However we do not like to do this and try to help you if possible sometimes by intensifying therapy. If all fails then there are various consequences including:
*Immediate Discharge
*Probation Clinic - Increased Therapy required, increased monitoring
No. FDA has approved Buprenorphine for treatment of opioid addiction. We do not prescribe Buprenorphine for off label use.
